Advanced Gravis Ultrasound Plug & Play version 3.0 with RAM. I think that it's 8MB of RAM, but I'm not sure anymore.

I am the Original owner. Stored in a climate-controlled computer parts collection for years.

Unfortunately I ...don't have a computer to test it in anymore, but it was working last I used it many years ago and all the capacitors look good so I expect that it's still working.
